Herbal recipes can be a useful, gentle accompanying measure in orthopedics - for example to relieve symptoms of joint and back problems or to support regeneration. In our practice in Hamburg (Dorotheenstrasse 48, 22301 Hamburg) we use herbs primarily in the form of teas, granules and decoctions. The selection is made individually, based on evidence and always as a supplement to proven conservative procedures such as physiotherapy, training therapy and pain and inflammation modulating measures. No method is promised across the board - what is crucial is a well-founded indication test, quality of the ingredients and careful support.

Overview: Why different dosage forms?

The form in which an herbal formulation is taken influences handling, taste, dosage and ingredient profile. Three tried and tested ways are: teas (infusions/infusions), granules (standardized extracts) and decoctions (decoctions). All have advantages and disadvantages. What matters is what goals we are pursuing (e.g. inflammation-modulating, relaxing, sleep-supporting), how suitable the preparation is for everyday use and which medications you are taking at the same time.

  • Teas: uncomplicated, mild, good to start with
  • Granules: precisely dosed, practical, reproducible
  • Decoctions: traditional, strong extract profile, but more complex

When can herbs be useful?

For orthopedic symptoms, individually tailored recipes can accompany the symptom-oriented treatment. They do not replace diagnostics or guideline-based therapies, but can be useful as a supplement in selected situations.

  • Osteoarthritis (e.g. knee, hip, hand): Support for pain and stiffness
  • Tendon and ligament problems (e.g. tennis elbow, Achilles tendon, plantar fascia): accompanying, phase-adapted recipes
  • Back and neck pain: muscle relaxing and sleep-supporting blends
  • Overuse syndromes in sport: regeneration support in coordination with the training and physio plan
  • Accompanying metabolic factors (e.g. weight regulation): metabolism-oriented mixtures as part of an overall plan

Important: Indication and form are always checked individually - especially in the case of previous illnesses, long-term medication, pregnancy/breastfeeding or planned surgery.

Teas (infusions/infusions): mild and suitable for everyday use

Teas are often the first step: they are easy to prepare, well tolerated and allow for gentle, continuous accompaniment. Depending on the herb, leaves, flowers or finely chopped roots are poured over with hot water and strained after a defined period of time.

  • Advantages: uncomplicated, inexpensive, easy to drink, flexible combinations
  • Disadvantages: lower concentration of active ingredients than extracts, daily preparation required
  • Typical use: 2-3 cups per day, as a course of treatment over several weeks

For those with sensitive stomachs, we can adjust the steeping time and temperature to increase tolerance.

Granules: standardized extracts for precise dosage

Granules are concentrated plant extracts that are dissolved in water (or warm tea) and taken as a drinking solution. They combine practical handling with reproducible quality - useful when clear dosages and suitability for everyday use are the priority.

  • Advantages: defined dosages, easy to take with you (e.g. work, traveling), more consistent quality
  • Disadvantages: Taste more intense, cost higher than tea, careful supply chains important
  • Typical use: 1-2 measuring spoons 1-3 times daily (depending on the recipe and goal)

We use granules with pharmacy quality and test protocols (e.g. for heavy metals, microbiology, pesticides).

Decoctions: traditional and rich in active ingredients

In the decoction, roughly chopped raw drugs are slowly boiled in water. This means that even poorly soluble ingredients can be extracted. Decoctions are traditionally anchored and come into question if a recipe is to use a wide range of plant components.

  • Advantages: high extraction, customizable (cooking time, order), strong customization possible
  • Disadvantages: time consuming, intense flavor, need for utensils and precise instructions
  • Typical use: daily portions, freshly prepared or stored in the refrigerator

We will decide together with you whether decoction makes sense - always taking into account the effort, suitability for everyday use and objectives.

Evidence: What is proven, what is plausible?

There is a growing but heterogeneous body of study on herbal therapy for muscular and joint-related complaints. Some research suggests possible relief of subjective symptoms (e.g., pain, stiffness) and better everyday function. At the same time, standardization, quality of the preparations and comparability of studies are a challenge.

  • Positive: individual clinical studies and empirical values ​​suggest complementary effects.
  • Please note: not all recipes have been tested; Results cannot be transferred across the board.
  • Consequence: individual indication, clear goals, follow-up checks and discontinuation if there is no benefit.

We integrate herbs exclusively as a building block in a multimodal, conservative treatment concept - no promises of healing.

Quality and safety come first

We work with pharmacies and verified manufacturers who check batches for identity and purity. In this way, we reduce risks caused by contamination and ensure reproducible quality.

  • Tests for heavy metals, pesticides and microbiological contamination
  • Transparent origin of raw materials and standardized extracts
  • Documented dosage recommendations and preparation instructions

Despite care, side effects can occur (e.g. gastrointestinal complaints, skin reactions). If you notice any abnormalities, please report them immediately.

Interactions and contraindications

Herbal preparations can interact with medications. A careful clarification is mandatory before we start a recipe.

  • Blood thinning/anticoagulation: individual herbs can influence blood clotting.
  • Blood sugar/blood pressure: possible additive effects – close monitoring necessary.
  • Liver/Kidney: Pre-existing conditions require particularly careful selection and dosage.
  • Pregnancy/breastfeeding: only very limited and after benefit-risk assessment.
  • Allergies/intolerances: Be aware of cross-reactions.
  • Operations: Herbs are usually paused 5-7 days before planned procedures - please consult.

Application, dosage and duration

The intake depends on the goal and tolerability. We often start low and increase when tolerated. Initial feedback is often possible after 2-4 weeks; A reliable assessment usually takes place after 6-8 weeks.

The exact composition depends on the diagnosis, findings and comorbidities. We often pursue several goals at the same time.

  • Inflammation modulation: For arthrotic attacks or tendinopathies
  • Muscular relaxation: neck/lumbar pain, stress-related
  • Microcirculation and warmth: for sensitivity to cold/start-up pain
  • Sleep and regeneration support: if you experience pain at night
  • Gastrointestinal protection: for sensitive digestion with analgesics
  • Metabolism-oriented: complementary to weight and training programs

Herbs work best as part of an overall plan. In our practice, depending on the findings, we combine with:

Which shape suits you?

  • Tea: when suitability for everyday use and mild accompaniment are the priority.
  • Granules: when exact dosage, quick preparation and constant quality are desired.
  • Decoction: if a more intensive, traditionally oriented extraction seems sensible and the preparation effort is acceptable.

Practical tips for application

  • Taste: Drink warm, sip slowly; For teas, if necessary with a little lemon. If possible, avoid sweetening and, if necessary, consider it sparingly and individually.
  • Travel/Office: Granules in portion doses; Thermo mug for tea/decoctions.
  • Storage: dry, protected from light, well closed; Preparations should be cooled and consumed promptly.
  • Breaks: Please consult if you have infections, gastrointestinal irritations or new medications.
  • Documentation: short weekly check (pain scale, sleep, activity) makes fine control easier.

Costs and reimbursement

The costs vary depending on the recipe, dosage form and duration. Statutory health insurance companies generally do not cover herbal preparations; Private payers sometimes reimburse on a pro rata basis after an individual review. We provide transparent information in advance about expected expenses.
