Orthopedic therapies – individual, evidence-based, patient-centered

We treat orthopedic complaints with a clear principle: conservatively first, minimally invasive and regenerative only when it makes technical sense. Our goal is to provide lasting relief from pain, restoration of function and a safe return to everyday life, work and sport. In our practice at Dorotheenstrasse 48, 22301 Hamburg, we will work with you to develop a structured treatment plan - transparent, understandable and oriented towards your goals.

Conservative and regenerative care: choose the right subpage.

Conservative orthopedics first: movement, relief, education

In the majority of cases, problems with the spine, joints, tendons and muscles can be controlled without surgery. The central building blocks are movement therapy, targeted training, manual techniques, ergonomic adjustments and clever pain management. We rely on active strategies that strengthen your self-efficacy and prevent relapses.

  • Physiotherapy and active training (strength, mobility, coordination)
  • Manual therapy, joint-friendly mobilizations, medical taping
  • Load control: “load management” suitable for everyday life and sports
  • Orthoses, bandages, insoles: limited in time and appropriate to the indication
  • Short-term drug therapy (e.g. anti-inflammatory, locally effective)
  • Pain education and ergonomic advice for the workplace and everyday life

We embed conservative therapies into a time-structured program. We define measurable stages (e.g. 4-6 weeks) and check goals such as pain reduction, mobility, resilience and return to activity.

Regenerative and interventional procedures: targeted rather than blanket

Regenerative procedures can be a useful addition in selected situations - especially in certain tendinopathy, irritation and wear conditions or in the early stages of osteoarthritis. We openly discuss the benefits, limitations and evidence. It is only used after basic conservative measures have been exhausted and only if there is a clear indication.

  • Autologous blood/PRP (ACP): for defined tendon irritations and early osteoarthritis
  • Hyaluronic acid injections: for relief from activated osteoarthritis
  • Targeted infiltrations (e.g. facet joints, bursa) – image-supported if possible
  • Shock wave therapy (ESWT): e.g. B. Plantar fasciitis, calcific shoulder, tennis elbow
  • TCM/acupuncture: in addition to pain modulation and muscle relaxation

Important: Not every method is suitable for every person. We provide information about effectiveness, risks, alternatives and costs. There cannot and should not be a guarantee for a certain success.

Careful diagnostics and clear treatment planning

A precise diagnosis is the basis of any effective therapy. In addition to anamnesis and clinical examination, if necessary, we use sonography, X-rays and – in cooperation – MRI. Functional tests and stress analyzes help to identify pain-causing mechanisms and treat them in a targeted manner.

Our therapy offering at a glance

Depending on the findings, we combine active and passive measures. The following overview supports orientation; Detailed information can be found on the subpages.

  • Conservative core components: physiotherapy & training, manual therapy, taping
  • Injection therapies: local and image-supported, with structured indications
  • Regenerative medicine: PRP/ACP, hyaluronic acid – case-specific and transparent
  • Shock wave therapy (ESWT): for selected tendon and attachment problems
  • TCM/acupuncture: in addition to reducing pain and improving function
  • Orthoses, bandages, insoles: temporary stabilization and relief
  • Pain management: anti-inflammatory, local, multimodal accompanying
  • Prevention & relapse prevention: education, home exercises, stress building

Process in our practice

We tailor the frequency of appointments to your symptoms, your goals and your everyday life. Transparency regarding benefits, risks and possible alternatives is a matter of course for us.

When you should seek medical advice quickly

  • Severe acute pain after trauma, visible misalignment, loss of feeling or strength
  • Pain at rest/night pain with redness, overheating, fever
  • Sudden swelling of a joint without an explainable cause
  • Increasing, therapy-resistant pain despite relief
  • Newly occurring bladder or bowel emptying disorders due to back pain

If there are warning signs like these, we ask that you come in for an examination as soon as possible or – depending on the severity – that you go to an emergency room for clarification.

Common symptoms – examples of treatment options

Treatment decisions depend on the cause, stage and personal goals. Examples of common indications and possible steps:

  • Back pain (unspecific): activation, educational pain management, training, short-term anti-inflammatory; In the case of facet syndrome, targeted infiltration may be necessary.
  • Knee osteoarthritis (early to moderate): weight and strain management, strength/coordination training, insoles/orthotics; occasionally hyaluronic acid or PRP.
  • Shoulder pain (impingement, calcification): posture/scapular stability, manual mobilization, rotator cuff training; If necessary, ESWT for calcified shoulder.
  • Achillodynia/Plantarfasiitis: eccentric training, relief, insoles; If necessary, ESWT or PRP if there is no improvement.
  • Tennis/golfer elbow: ergonomic adjustment, eccentric-isometric training, tape; if persisting: ESWT or injection.
  • Hip (bursitis, tendinopathy): load control, glute training, manual therapy; In the case of irritation, targeted infiltration may be necessary.

What you can do yourself

  • Regular, pain-adapted movement instead of rest – “as much as is good”
  • Home exercises (strength, mobility, balance) 3–5 times per week according to instructions
  • Stress management: small steps, realistic milestones, diary
  • Ergonomics: Optimize the workplace and everyday life (e.g. lifting, sitting, breaks).
  • Sleep and stress management: promote pain control and regeneration
  • Weight management for osteoarthritis: every kg less relieves joints noticeably

Costs and transparency

Frequently asked questions

This depends on the diagnosis, duration of the symptoms and training consistency. We often see initial progress within 2-6 weeks. We agree on clear checkpoints and adjust therapy if necessary.

If conservative basic measures are inadequate and the diagnosis makes targeted infiltration or a regenerative measure plausible. Before each injection, we discuss the benefits, risks, alternatives and evidence.

PRP is a prepared autologous blood preparation with growth factors, which is primarily used a. in tendinopathy and early osteoarthritis is discussed. Hyaluronic acid acts as a lubricant and buffering agent in the joint and is used to relieve symptoms in osteoarthritis. Not every method suits every complaint.

This varies depending on the fund and service. Conservative standard measures are usually adopted. Regenerative procedures are often self-pay services. We will inform you transparently in advance about the expected costs.

Like any intervention, injections carry risks, e.g. E.g. infection, bleeding, temporary increased irritation or, rarely, allergic reactions. We minimize the risk through sterile technology, targeted indications and information.

Conservative therapy programs usually last 4-12 weeks. Series of injections vary depending on the procedure and findings (e.g. 1-3 sessions). We align the frequency with your course and evidence recommendations.

Usually early, adjusted for pain and function. We support the gradual build-up of stress and define individual criteria for returning to training and competition.

Yes. Please bring preliminary findings with you. We review diagnosis and treatment options and discuss conservative alternatives and, if necessary, minimally invasive options.
