Ear acupuncture
Ear acupuncture is a specialized form of acupuncture in which targeted points on the auricle are treated. These points can be used to positively influence functions of the musculoskeletal system, pain and vegetative side effects such as sleep or stress regulation. In our orthopedic practice in Hamburg-Winterhude, we use evidence-based and careful auricular acupuncture as a component of conservative therapy - always individually tailored and as a supplement to measures such as physiotherapy, exercise therapy and manual medical procedures.

What is ear acupuncture?
Ear acupuncture (auricular medicine) is based on the idea that the entire body is reflected in the form of a somatotopy in the auricle - in simple terms, like a small, upside-down embryo. Stimuli at certain points on the ear can influence the autonomic nervous system and pain-modulating networks via nerve tracts and reflex arcs.
Points that correspond to the spine, large joints (knee, shoulder, hip) and myofascial structures are particularly relevant from an orthopedic point of view. Depending on the findings, they are stimulated with fine disposable needles, smaller permanent needles (which last for a few days) or needle-free using a laser.

Differentiation from body acupuncture
Classic body acupuncture stimulates meridians throughout the body. Ear acupuncture works exclusively on the auricle and uses reflex zones, not primarily meridians. Both procedures can complement each other usefully: While body acupuncture affects systemic patterns, ear acupuncture can quickly modulate acute pain points and vegetative imbalances.

When can ear acupuncture be useful?
We primarily use ear acupuncture in orthopedics as a supplement when there is pain, muscle tension or functional limitations and non-medicinal, gentle support is desired. It does not replace diagnostics or guideline-based basic therapy, but can often usefully accompany these.
- Chronic and subacute low back pain, functional back pain
- Knee problems (e.g. pain caused by arthrosis) as a supplement to exercise therapy
- Shoulder and neck pain, myofascial trigger points
- Tendinopathies (e.g. Achilles tendon, epicondylitis) as part of a multimodal concept
- Post-operative or post-traumatic pain conditions (adjuvant, after medical approval)
- Accompanying relaxation in pain-associated sleep disorders or stress reactions
Important: Not every complaint is suitable for ear acupuncture. We always examine the indication individually – based on your findings, your goals and the available evidence.
Process in our practice
The treatment process is structured and designed for safety and traceability. We use sterile disposable needles and work according to hygienic standards. The treatment is usually well tolerated.
We typically recommend a series of 4-6 sessions every week. Depending on the course, refreshments are possible. The number and intervals depend on the complaint and reaction - we do not make blanket promises.
Evidence, benefits and limitations
There are growing but heterogeneous studies on ear acupuncture. Studies show possible relief for certain pain indications, especially in multimodal concepts. Placebo effects, expectations and the quality of implementation play a role. A guaranteed effect cannot be seriously guaranteed.
In Germany, standard health insurance coverage for acupuncture is limited to certain body acupuncture indications (chronic lumbar spine pain, gonarthrosis). Ear acupuncture is usually a self-pay or private health insurance service. We provide transparent advice on opportunities, risks and alternatives.

- Limits: structural damage or acute emergencies require other treatment options
Safety, risks and contraindications
Ear acupuncture is considered safe when used professionally. Side effects are usually mild and temporary. We clarify previous illnesses, medications and special situations (e.g. pregnancy) in advance.
- Frequent, mostly mild reactions: brief puncture pain, small bleeding, hematoma, feeling of warmth, tiredness
- Less common: circulatory reactions (dizziness, nausea) - therefore rest and follow-up
- With permanent needles: local irritation or infection possible - careful care and immediate report if redness/pain occurs
- Allergies: rarely to metals – we use suitable materials
- Relative contraindications: severe coagulation disorder, high-dose anticoagulation (individual consideration), local skin inflammation of the ear
- Pregnancy: certain points are avoided; Benefit-risk assessment
For cardiac pacemakers or implanted electronic devices, we do not use electrical stimulation on the ear. Pure needling is usually possible unless there are other reasons against it.
Preparation and aftercare
- Eat lightly and drink enough before your appointment, avoid alcohol
- Comfortable clothing, free access to the ear (remove jewelry)
- Bring a list of medications, especially blood thinners and painkillers
- After the session: rest briefly, avoid heavy endurance exercise on the day of treatment
- For permanent needles: keep the point clean and dry; Avoid swimming pool/sauna for a few days
- Warning signs (severe redness, swelling, fever): please report as soon as possible
Combinations and alternatives
Depending on the findings, we combine ear acupuncture with other acupuncture techniques or conservative measures. The aim is a tailor-made, holistic and at the same time evidence-oriented therapy.

- Trigger point and myofascial acupuncture: precise relaxation of overactive muscles
- Cranial acupuncture (YNSA): neurofunctional supplement for chronic pain
- Electroacupuncture: additional stimulation for selected indications

Treatment plan, duration and costs
A single ear acupuncture session usually lasts 20-30 minutes. If we use permanent needles, they usually remain in place for a few days and are removed during the check-up session. The initial treatment cycle often includes 4-6 appointments; If the response is good, longer intervals or refreshers are possible.
Reimbursement varies: Statutory health insurance companies generally do not cover ear acupuncture. Private health insurance companies reimburse proportionately depending on the contract. For self-payers, we provide transparent information in advance about the expected costs. We would be happy to clarify any questions about costs personally before treatment begins.
