What are Moxa Cones?

Moxa cones are made from finely processed wormwood that is pressed into small, easy-to-form cones. Controlled burning creates an even, deeply penetrating heat. This is used precisely on skin areas or acupuncture points without damaging the skin - in our practice only as an indirect application with a protective layer.

  • Application: indirectly via a protective carrier (e.g. ointment film, special moxa pads) to avoid burns

Moxa cones differ from moxa sticks (also called moxa cigars) in that they have a very focused, selective heating effect. Rods allow more extensive, circular heating; Cones have a targeted effect on individual points.

Active principle: heat, tissue and nerves

The therapeutic principle of Moxa cones is based on well-dosed heat stimulation of superficial tissue. On a physiological level, blood circulation and metabolism are locally stimulated, muscle tone and myofascial tension can decrease. The heat can modulate pain-processing nerve mechanisms and can therefore be perceived as providing short-term relief.

Areas of application in orthopedics (with clear indication)

We use moxa cones in a complementary and targeted manner - especially when symptoms are aggravated by cold, when muscular tension is at the forefront, or when gentle, deep heat seems useful. Acute inflammation or recent injuries, however, are exclusion criteria.

  • Myofascial tension in the neck, shoulder and lumbar areas
  • Non-specific back pain with sensitivity to cold
  • Degenerative joint problems (e.g. knees) in cold, non-inflammatory phases
  • Chronic tendon and insertion tendinopathies outside of acute irritation conditions

A medical examination is always crucial. We check whether heat application makes sense and is safe and incorporate moxa cones into an individual, active therapy concept - e.g. B. with exercise therapy, manual medical measures, education and, if necessary, acupuncture.

Process in our practice in Hamburg

Depending on the indication, a session usually lasts 15-30 minutes. Several small cones can be used one after the other per point to dose the heat. A characteristic herbal scent is noticeable during application; the room is ventilated accordingly.

Safety, side effects and contraindications

Moxa is a heat application and must be dosed carefully. We only use indirect processes with a protective layer. However, as with any heat therapy, side effects are possible.

  • Possible side effects: temporary redness, feeling of warmth, rarely blistering or slight irritation of the skin
  • Smoke/Swell: Smell of ragweed; If the room is sensitive, it is particularly ventilated
  • Be careful with respiratory diseases: e.g. B. Asthma – please specify in advance
  • Not suitable for: acute inflammation, recent injuries, fever, open skin areas, sensitive neuropathies, severe circulatory disorders, known contact allergies, unclear swelling
  • Special features: During pregnancy, certain regions/trigger points are avoided; we advise individually

Please inform us about blood thinners, diabetes with sensitivity disorders, known allergies or implants. Metal implants are generally not a problem as long as there is no direct heat.

Importance in the conservative treatment concept

Moxa cones do not replace active therapy, but can be useful as a component in a multimodal, conservative approach. Heat provides relief for some musculoskeletal problems; the specific evidence on moxibustion is mixed and methodologically limited. We only use the method if the expected benefit outweighs the risk and if it fits into your overall concept.

We decide which form is suitable based on the examination and objectives. For sensitive skin, we prefer indirect procedures with the lowest possible irritation intensity.

Preparation and follow-up

  • Before the session: Please inform us about any previous illnesses, medications and known allergies.
  • Clothing: Comfortable, easily accessible clothing makes treatment easier.
  • After the session: Take a short rest, drink enough, and initially avoid extreme cold/dampness on the treated area.
  • Training: Light mobilization/stretching can support the effect; Intensive training immediately afterwards only after consultation.

Evidence: What is certain – what is not?

There are studies that describe short-term effects of moxibustion for certain musculoskeletal complaints. However, the overall quality of the evidence is limited and heterogeneous, and not every patient benefits. We see moxa cones as an option as part of a pragmatic, conservative approach - transparent, without promises of salvation and with realistic goals.
