Hybrid therapy
Hybrid therapy combines proven conservative measures with selected regenerative procedures to relieve pain, improve function and specifically support the body's own healing. The focus is on precise diagnostics, a clear step-by-step plan and the individually sensible combination - without blanket promises. In our orthopedic practice at Dorotheenstrasse 48, 22301 Hamburg, we advise you transparently and based on evidence.

What does hybrid therapy mean?
Hybrid therapy is not a single procedure, but a structured treatment approach: Depending on the findings, we combine physiotherapy and training therapy measures with targeted injections (e.g. hyaluronic acid, inflammation-modulating preparations), biological procedures (e.g. PRP) and accompanying methods such as shock wave therapy. The aim is to support tissue healing, reduce pain and restore resilience - always with clear indications and realistic expectations.
- Individual: therapy combination based on anamnesis, examination and imaging
- Gradually: From conservative, basic to selected regenerative options
- Precise: Ultrasound-targeted injections, functional tests, follow-up
- Evidence-conscious: Benefits and limitations are openly discussed
Who is hybrid therapy suitable for?
Hybrid therapy is suitable for patients with musculoskeletal complaints for whom a single measure is unlikely to be sufficient or has already been exhausted. Typical areas of application are:
- Tendon and ligament problems (e.g. tennis elbow, golfer's elbow, Achilles tendon problems, patellar tendinopathy)
- Fascia and muscle irritation (e.g. plantar fasciitis, myofascial trigger points)
- Early to moderate osteoarthritis of individual joints (e.g. knee, hip, shoulder, thumb saddle joint) - depending on the findings
- Overuse syndromes during sports and everyday life (e.g. runner's knee, shin splints)
- Certain intervertebral disc and nerve irritations with conservative treatment options
Whether combination therapy makes sense is decided after a clinical examination, possibly ultrasound/MRI and weighing up goals, risks and alternatives.
Useful combinations in orthopedics
Not every combination is automatically better. We choose building blocks that complement each other mechanistically and fit the individual healing goal.
- PRP + training therapy: Regenerative stimulation with targeted stress build-up to reorganize collagen structures (e.g. in tendinopathies).
- PRP + hyaluronic acid (scheduled or coordinated): Inflammation modulation and viscosity support for certain osteoarthritis and tendon sliding problems.
- Shock wave therapy + eccentric training: promoting local blood circulation and tissue remodeling, flanked by structured exercises.
- Ultrasound-targeted injection + manual therapy: precise deposition on the target tissue with accompanying mobilization.
- Inflammatory modulation (e.g. low-dose local preparations) + activation: reduce discomfort to increase training tolerance.
- Ergonomics and running style coaching + therapy: Optimize everyday and sports factors to avoid relapses.
PRP means platelet-rich plasma from your own blood. Hyaluronic acid is a viscoelastic gel that can improve lubrication in joints. Both procedures are not a replacement for exercise therapy, but can – if indicated – be part of a hybrid concept.
Diagnostics and treatment planning
Hybrid therapy begins with a careful diagnosis to identify the main source of pain and influenceable factors.
- Anamnesis: course, stress profile, previous illnesses, medication
- Clinical examination: functional, stability and stress tests
- Imaging: Soft Tissue/Joint Ultrasound; MRI/X-ray if necessary
- Risk profile: coagulation, signs of infection, individual contraindications
- Goal definition: pain, function, sports or professional goal
The findings are used to create a step-by-step plan with clear milestones, checkpoints and realistic time horizons.
Process: step by step
Rest phases after injections, dosage of stress and any breaks from work or sport are individually tailored.
Evidence: opportunities and limitations
There is good evidence for many basic measures of conservative orthopedics (education, activity control, targeted strengthening). When it comes to regenerative procedures, the study situation is heterogeneous depending on the indication: for tendinopathies, some studies show advantages of PRP or shock wave, other results are neutral. In the case of osteoarthritis, studies sometimes report temporary improvements with hyaluronic acid or PRP, sometimes with no clear additional benefit compared to standard measures.
A hybrid therapy attempts to combine the strengths of the individual components. Nevertheless, the effect and duration vary from person to person. We discuss benefits, uncertainties and alternatives transparently - and set verifiable goals together.
Risks and side effects
Even if done carefully, side effects can occur. We provide detailed information in advance and minimize risks through sterile technology and precise application.
- Temporary pain, swelling, or bruising at the injection site
- Irritation reaction of the tendon or joint in the first few days
- Rare: infection, nerve irritation, vascular injury, allergic reactions (especially with non-autologous preparations)
- Insufficient effect despite correct use
Important: After injections, the load should be increased gradually. Overloading immediately afterwards increases the risk of setbacks.
Contraindications and caution
- Acute infections, fever, inflammatory skin lesions in the treatment area
- Certain clotting disorders or blood thinning (depending on the procedure)
- Uncontrolled systemic diseases
- Pregnancy/breastfeeding: individual consideration depending on the method
- Specific allergies/intolerances (for non-autologous preparations)
We check contraindications in the consultation and adapt the therapy accordingly.
Preparation and follow-up treatment
- Preparation: Bring a list of medications (including blood thinners, NSAIDs), previous findings/pictures, sports and everyday goals.
- After injections: 24-48 hours of protection of the target area, then dosed increase in load according to plan.
- Training: Eccentric and isometric exercises are often a central component.
- Accompanying factors: sleep, nutrition, stress – note the influence on regeneration.
- Checks: Attend appointments to assess progress and adjust plans.
Costs and reimbursement
The costs of a hybrid therapy depend on the components chosen (e.g. number of injections, shockwave sessions, controls). Statutory health insurance companies do not cover all services; selected procedures are considered self-pay services. Private insurance companies reimburse depending on the contract.
Before you start, you will receive a transparent cost breakdown. We advise which options make medical sense and which alternatives exist.

Exemplary application scenarios
- Achilles Tendinopathy: Eccentric Training + Shockwave; If persistent, supplement with PRP, then gradually increase the load.
- Knee osteoarthritis (early–moderate): education, weight and activity management, muscle building; Depending on the findings, a series of injections (e.g. hyaluronic acid or PRP) with accompanying physiotherapy.
- Tennis elbow: load management, eccentric exercises, possibly shockwave; Targeted injection for stubborn cases.
- Plantar fasciitis: stretching and strengthening program, insoles advice; If there is no improvement, additional ESWT or PRP.
These examples do not replace individual diagnostics. The specific combination is always chosen based on the patient and findings.
