Our holistic and integrative diagnostics in Hamburg combine guideline-oriented orthopedics with modern functional analysis, imaging and targeted laboratory and regulatory diagnostics. The aim is to provide reliable findings and understandable explanations - as the basis for an individual, predominantly conservative treatment plan. What does “holistic & integrative” mean in orthopedics?

Holistic means: We look at people in their context – biomechanical, inflammatory-metabolic and neuroregulatory. Integrative means: We combine proven clinical examinations and imaging with additional analyzes if they answer a specific question. Not every patient needs all the building blocks. What matters is the relevance to your complaints.

  • Biomechanics and tissue: joints, tendons, ligaments, fascia, muscles
  • Nerve and muscle function: coordination, activation, protective tension
  • Inflammation and metabolism: systemic and local factors
  • Hormone and regulatory axes: e.g. B. Stress and thyroid axis (only if appropriate)

  • Environmental and injury history: previous surgeries, scars, mechanisms of accident

Our practice works on the “clinical question first” principle. Every examination should help to confirm or rule out a plausible cause - and pave the way to safe, predominantly conservative therapy.

Who is integrative diagnostics suitable for?

Integrative diagnostics is particularly useful if symptoms recur, are complex or persist despite initial attempts at therapy. A structured view of function, tissue and metabolism can also be helpful for physically active people who want to specifically control stress and regeneration.

  • Chronic or recurring back, shoulder, knee or hip pain
  • Tendon problems (e.g. Achilles tendon, patellar tendon, tennis elbow) with an unclear course
  • Early stages of osteoarthritis with a need for prevention and everyday strategies
  • Sports injuries with delayed healing or repeated re-inflammation
  • Muscular imbalances, feelings of instability, loss of strength or coordination
  • Complaints with a possible systemic component (e.g. fatigue, sleep disorders, weight fluctuations) – only if there is a suitable clinical hypothesis
  • Clarification before planned training changes, running style or workplace adjustments

Important: Warning signs such as fever, pain at night when resting, paralysis, sensory disturbances, significant swelling or acute injuries with loss of function must be clarified promptly and conventionally. We control the diagnostics according to guidelines and work with other specialist disciplines if necessary.

Building blocks of our diagnostics – modular and targeted

The building blocks are combined individually. The basis is always anamnesis and clinical examination. Depending on the question, we add functional and imaging procedures as well as laboratory and regulatory analyses.

  • Clinical examination and functional check: posture, mobility, strength, coordination
  • Ultrasound diagnostics: Dynamic assessment of tendons, muscles, bursa and surface structures
  • Elastography: Additional information on tissue elasticity in selected tendon and muscle topics
  • Cone beam CT (CBCT) for extremities: low-dose 3D for detailed bony questions (only if strictly indicated)
  • Laboratory diagnostics: inflammatory markers, metabolism, possibly micronutrient status if the hypothesis is suitable
  • Micronutrient & vital substance analysis: targeted, if clinically relevant
  • Hormone & regulation diagnostics: e.g. B. Thyroid, stress axes; only if there is reasonable suspicion
  • Neuro & muscle functional diagnostics: activation patterns, neuromuscular control, fatigue

For each component, we provide transparent advice about the benefits, limitations, possible risks (e.g. radiation exposure in CBCT) and costs or reimbursement. Not all services are included in the health insurance plan; we will clarify this in advance.

Benefits, evidence & limitations

Integrative diagnostics is not “more is better”, but rather “as much as necessary”. Investigations are intended to improve decisions. We are guided by guidelines, studies and clinical plausibility. Many procedures such as ultrasound and standardized laboratory parameters are well established. Other components - such as certain micronutrient or regulatory analyzes - can be useful if the anamnesis suggests this, but are not evidence-based or reimbursable in all cases.

  • Radiation-free procedures have priority; CBCT only if the question is clear
  • Every additional diagnosis requires a clinical hypothesis
  • Avoidance of chance findings through economical, targeted selection
  • Transparency regarding significance, limitations and alternatives

Diagnostics does not replace therapy – it makes it more precise. There are no guarantees. We discuss realistic goals and initially focus on conservative, low-risk measures.

No. After anamnesis and examination, we only select the components that make sense for your problem. Clinical examination and ultrasound are often sufficient. Further tests are only carried out if there is a clear indication.
